Whenever you place an order for blood tests you'll be given the opportunity at checkout to also pay for phlebotomy at any one of our participating clinics or a nurse visit. If you want to buy more to accompany multiple tests you have bought, or if you want to buy phlebotomy again after a failed collection, or if you are upgrading from a Finger-Prick Kit then this is where you can buy extra phlebotomy sessions. You'll be given the opportunity to assign them to tests once you have placed your order. PLEASE NOTE THIS IS ONLY AVAILABLE AS AN ACCOMPANYING TEST FOR AN EXISTING ORDER OR FOR EXTRA PHLEBOTOMY SESSIONS AS DETAILED IN OPTIONS 1, 2 AND 3 BELOW. THIS CANNOT BE USED TO BOOK PHLEBOTOMY SESSIONS FOR TESTS ORDERED FROM ANOTHER MEDICAL PRACTICE.

If you had not yet completed the Patient form for your previous Order Please enter the Order Number starting KITREQ in the box provided, and then once THIS Order has been completed, the patient form (link in your previous order email) will now show this new phlebotomy option as available.

OPTION 2. If however you had completed the Patient form already and received a Confirmation email with an LR number, you should now enter the LR number that you want this phlebotomy option to be added on ie LRCP123456-0 . Once THIS Order goes through, that Laboratory Request will be cancelled and a new one created, showing this new phlebotomy method.

OPTION 3 **For Paid Phlebotomy options Only, does not apply for arrange your own phlebotomy kits** If you are ordering this phlebotomy session as extra paid phlebotomy to the phlebotomy option you will choose in checkout because you are ordering multiple tests for different patients or submission times, please enter " This Order "

However, if you are ordering blood tests for multiple people on the same order you can put order extras here. We'll help you assign which tests and which sample collection options go with which patient once you have placed your order.

You can also order this Circle Health Hospital Attendance as an Add-on if you have had a failed sample collection from either a Finger-Prick Blood Test or a Blood Draw, or originally didn't request one at checkout.
